Position Description

The Community Support Services Coordinator is a member of Kenora Chiefs Advisory’s Community Health and Cultural Services team. This position will provide liaison, support and case management for the First Nation Home Maintenance & Repair and supporting the Homemaker Programs, ensuring the needs of the elderly are being met

Position Title:                 Community Support Services Coordinator

Department:                   Community Health and Cultural Services

Term:                               Full time

Location:                         240 Veterans Drive, 3rd Floor, Kenora, Ontario

Reports to:                      Director of Services–Community Health and Cultural Services

Summary of Duties

  • Work as part of the Health Team
  • Provide direction and support to the Community Coordinators in the First Nations
  • Complete client assessments on home maintenance and repair and homemaker clients
  • Advocate and navigate services for clients and health care providers in communities and First Nation members in Urban area
  • Network with local specialized service providers
  • Connect and work with community health service providers to ensure efficient program delivery
  • Arrange quarterly meetings with community coordinators from participating First Nations
  • Ensure accurate reporting of statistics and funding
  • Complete the necessary research and proposals for expansion programs, training and services
  • Obtain and complete quarterly reports and process monthly payments
  • Other duties as required

Qualifications

  • Completion of a post-secondary program that relates to this field or equivalent combination of training and experience
  • Current knowledge and understanding of Federal and Provincial health programs
  • General understanding of First Nation culture, values and beliefs
  • Completion of a post-secondary program that relates to this field or equivalent combination of training and experience
  • Current knowledge and understanding of Federal and Provincial health programs
  • General understanding of First Nation culture, values and beliefs
  • Fluent in Ojibway language is a definite asset
  • Experience working in a First Nation health organization

  Position Requirements

  • Valid driver’s license and vehicle
  • Must be willing to travel when required
  • Must be willing to relocate to Kenora,ON
