Position Description

The Community Diabetes Educator is part of the Health Promotion and Prevention team at Kenora Chiefs Advisory. This position will provide culturally appropriate awareness and support to community members in KCA’s member First Nations communities. The goal of this program is to provide individuals with the knowledge and skills to achieve their optimal health

Position Title:                 Community Diabetes Educator

Department:                   Health Promotion and Prevention

Salary:                              Per Annum, after successful probationary period

Term:                               Full time – 1 FTE

Location:                         240 Veterans Drive, 3rd Floor, Kenora, Ontario

Reports to:                      Director of Service-Health Promotion and Prevention

Summary of Duties

  • Provide education, awareness and support in areas of nutrition, physical activity and chronic disease management
  • Develop, deliver and evaluate diabetes education and prevention initiatives in collaboration with the nine Kenora Chiefs Advisory member communities
  • Facilitate group education sessions, support groups, workshops and community screenings
  • Maintain close collaborative working relationships with other health care providers
  • Participate in community health events
  • Ensure education provided is evidence-based and culturally appropriate

Qualifications

  • Degree/Diploma with current registration in related health field (RN, RPN, RD)
  • Experience in diabetes education and health promotion
  • Diabetes Educator Certification is an asset
  • Demonstrated commitment and knowledge of community-based health care
  • Excellent written, oral, interpersonal and presentation skills
  • Excellent computer skills
  • Knowledge of the Anishinaabe culture and language
  • Ability to work independently and collaboratively as a member of a multidisciplinary team

  • Valid driver’s license and vehicle
