Position Description:
Reporting to the Chief Operating Officer, the Implementation & Data Specialist will work as part of the Strategy & Innovation department, supporting the implementation of the Homelessness & Addiction Recovery Treatment (HART) Hub project. This role will work closely with partners across sectors to implement the HART Hub initiative. The ideal candidate brings experience implementing complex, community-based health initiatives, applying mainstream best practice and Indigenous approaches to care.
Duties and Responsibilities:
Implementation Support
· Work as an active member of the project team to support all aspects of the HART Hub implementation.
· Support KCA and partner organizations to co-create a shared service model that reflects community needs, values, and priorities.
· Facilitate the identification, documentation, and implementation of coordinated service pathways and referral processes across partner organizations.
· Build a strong understanding of existing mental health, addictions, housing, and related services, and help apply that knowledge to improve access, equity, and continuity of care.
· Engage regularly with partners and other stakeholders to integrate their knowledge into planning and implementation activities.
· Support system and service change efforts through problem-solving, capacity-building, and collaborative planning.
· Apply an equity, anti-racism, anti-oppression, and cultural safety lens to all implementation work.
Data & Reporting
· Design and manage data collection processes that enable multiple partner organizations to contribute consistent, high-quality data to the lead agency (KCA).
· Analyze data from across the system to develop clear, accurate reports for funders (e.g., Ministry reporting) and leadership.
· Translate data insights into meaningful feedback for service partners to identify trends, gaps, and opportunities for service improvement.
· Create user-friendly data visualizations and summary reports that support learning, reflection, and continuous improvement within the HART Hub.
Education & Experience:
· A post-secondary degree in a relevant field such as Health, Social Services, Education, Business Administration, Statistics, or Information Management.
· Minimum of 3 years of experience in the field of healthcare, social services, or a related sector.
· Equivalent combinations of education and professional experience will be considered.
Qualifications:
· Experience supporting the implementation of complex, community-based health or social service initiatives.
· Strong interpersonal and communication skills, with a demonstrated ability to engage stakeholders, facilitate dialogue, and navigate complex challenges.
· Excellent organizational and time management skills, with the ability to manage multiple priorities and meet deadlines.
· Ability to work both collaboratively as part of a team and independently with minimal supervision.
· Commitment to data-informed decision-making, grounded in both mainstream and Indigenous approaches to evidence-based practice.
Skills:
· Strong analytical and critical thinking skills, with experience using data to support planning, quality improvement, and accountability.
· Demonstrated ability to produce clear, accessible reports and data visualizations for a range of audiences.
· Understanding of Indigenous approaches to health and wellness, including cultural safety, trauma-informed care, and holistic well-being.
· Excellent written and verbal communication skills, including the ability to synthesize and convey complex information effectively.
Position Requirements:
· Current satisfactory Criminal Reference Check with Vulnerable Sector Check;
· Valid driver’s license and vehicle;
· Willing to travel when required;
· Understanding of the Ojibway language is an asset.
