Position Description:

The Homelessness, Addiction, and Recovery Treatment (HART) Hub is a collaborative initiative designed to provide a seamless continuum of care through a culturally responsive and healing-centered model. Tailored to meet the needs of individuals facing homelessness, addiction, and mental health challenges, the Kenora HART Hub serves as a vital anchor for support within the Kenora Geographical Area and surrounding communities. By delivering coordinated, compassionate, and evidence-based services, the HART Hub ensures equitable access to care. This approach integrates holistic and culturally grounded practices, to promote stability, healing, and long-term recovery for members who are most vulnerable within our communities.

Reporting to the Team Lead, the Adult Case Manager will be on rotation through the Kenora HART Hub, as well as multiple sites in First Nation Communities. The Case Manager will deliver culturally safe, trauma informed, Transitional Support, clinical case management for mental health and addictions service.  This position includes supporting an individual’s immediate needs by navigating homelessness, mental health, addictions, referral pathways, and aftercare while maintaining accurate client records.  These services will be delivered within the policy guidelines of Kenora Chiefs Advisory and the HART Hub mandate. The Adult Case Manager will work within the HART Hub to help deliver and improve the services offered to the relevant communities.

Starting Salary:         70,000.00

Duties and Responsibilities:

Case Management & Transitions

  •  Conduct intake and admissions for walk ins, collecting referral information, and consulting with a Team Manager around the need for a crisis;
  • Develop an Initial Intake Assessment (psychosocial assessment) at first contact and create an individualized Care Plan within 30 days for each client;
  • Provide clients with adequate and seamless transitions through different services, referrals, appointments, etc.
  • Minimize the impacts clients experience navigating multiple and at times complex systems.
  • Supporting clients to use their voice and be heard when any planning occurs;  
  • Provide ongoing case management, including crisis intervention, advocacy, facilitating referrals to any required needs, and supporting individual’s transitional pathways they navigate on their wellness journey;
  • Collaborate with multiple care providers, addiction specialists, supportive housing providers, and cultural healing and wellness providers;
  • Provide information, education, capacity building and guidance to clients regarding mental health and addictions;
  • Provide counseling to clients on specific ways to recognize triggers, and coping strategies;
  • Provide information, education, capacity building, and support to clients to manage their overall well-being;
  • Monitor clients’ mental status and behavior for decline and take appropriate action;
  • If requested by client, facilitate traditional health/healing services.  Note:  This could be an alternative to, or in conjunction with contemporary methods;
  • Facilitate therapeutic groups. This could include; in community sharing circles, therapeutic debriefing sessions, mental health support groups, Life skills support, relapse prevention group work, etc.;
  • Participate in inter-disciplinary case conferences;
  • Keep accurate, timely client contact records in accordance with program requirements;
  • All data entry requirements should be completed on a weekly basis;
  • Assist and respond to crisis and post-crisis situations as required and complete individualized safety plans;
  • Pre and Post Treatment care is required for individuals struggling with mental health and addictions while ensuring support within the continuum of care and recovery;
  • Supporting multiple levels of re-integration after incarceration, following treatment or detox;
  • Liaise closely with NDAADAP in First Nation Communities
  • Enhance individuals’ stability with incorporating cultural support where required;
  • Alert the Team Lead and/or the Director to all critical cases and situations.

External Referrals & Support

  • Facilitate linkages to needed services within the broader service community for example: housing, treatment/detox centers, safety/stabilization beds;
  • Develop a strong relationship within the HART Hub, and community resources;
  • Encourage and support clients in building their own social lives, independence and support systems to reduce dependency on the Case Manager;
  • Advocate for client and coordinate case management meetings with relevant partners;
  • Facilitate access to Anishinaabe knowledge, traditions and cultural beliefs in the healing process (if appropriate);
  • Facilitate referrals to OTN for consultations and assessments.

Monitoring & Evaluation

  • Maintain statistics required by the Ministry and the Program with regard to qualitative and quantitative aspects of the Program.
  • Contribute to the evaluation of service with respect to its effectiveness in meeting the community needs and achieving our mandate

Qualifications

  • University degree or college diploma from a recognized post-secondary institution (Psychology or social work);
  • 3-5 years Case Management Experience
  • Knowledge of the Anishinaabe culture and experience in working in First Nation communities;
  • Knowledge and understanding of Intergenerational Trauma;
  • Experience in counselling adults and families in all areas of mental health and addictions;
  • Experience providing support to adults with significant trauma, mental health, and substance use challenges;
  • Ability to deal with crisis and post-crisis situations;
  • Ability to work independently and in a team setting;
  • Excellent networking, interpersonal, presentation and communication (written & oral) skills;
  • Experience in engaging with various systems such as but not limited to:  ODSP, CPP, Legal Aid, housing systems, health systems, criminal justice systems etc.;
  • Proficiency in the use of computers and various software applications;
  • Demonstrated ability to work with multi-disciplinary teams;
  • Experience working with and knowledge of Indian Residential School survivors.

Position Requirements

  • Valid driver’s license and vehicle;
  • Willing to travel to all Treaty #3 First Nation communities as per program needs;
  • Current satisfactory criminal reference check with vulnerable sector search;
  • Understanding of the language of the Anishinaabe language (Ojibway) is an asset.
  • Knowledge of regional mental health and social supports, as well as traditional healing would be considered an asset
