To obtain your FIT Kit to complete your colon screening in your community serviced by Ogimaawabiitong – Kenora Chiefs Advisory (KCA), here is how you can be assessed for eligibility:

Grassy Narrows Health Centre: call 807-925-2850

  • Make an appointment with your KCA physician or WNHAC nurse practitioner.
  • Return your completed FIT Kit to ISC nurse at the Grassy Narrows Health Centre.

Naotkamegwanning Health Centre: call 807-226-1026

  • Make an appointment with your Community Health Nurse (CHN) or KCA physician or WNHAC physician.
  • Return your completed FIT Kit to the CHN at the Health Centre.

Niisaachewan Health Centre: call 807-548-5876

  • Make an appointment with your KCA primary care nurse or physician or WNHAC nurse practitioner.
  • Return your completed FIT Kit to the primary care nurse at the Health Centre.

NWA #33A or Dogpaw Health Centre: call 807-226-9890

  • Make an appointment with your KCA physician or WNHAC nurse practitioner.
  • Put your FIT Kit in the mail within 2 days of completing it.

  • Put your FIT Kit in the mail within 2 days of completing it.

  • Put your FIT Kit in the mail within 2 days of completing it.

  • Make an appointment with your KCA physician or nurse practitioner.
  • Return your completed FIT Kit to ISC nurse at the Wabaseemoong Health Centre.

Wauzhushk Onigum Health Centre: call 807-548-6340

  • Make an appointment with your KCA primary care nurse or physician or nurse practitioner.
  • Return your completed FIT Kit to the primary care nurse at the Health Centre.

KCA Community Members Without a Health Care Provider: contact 811 (Health811) to be assessed and receive a FIT Kit in the mail.

  • Put your FIT Kit in the mail within 2 days of completing it.
